At The Heights we take the well-being of our residents very seriously and we want everyone at our facility to feel as comfortable here as they would in their own homes. We’ve recently renovated patient rooms to reflect this commitment to comfort and convenience.

Admissions

Our admissions director will walk you through the admissions process and help you get acclimated to your new environment. These first steps can be the most confusing, so our admissions staff is prepared to carefully guide you and answer your questions.

Dining Experience

Creating an enjoyable dining experience for our residents is a priority at The Heights. While offering a wide variety of tasty and nutritious menu choices, our dieticians are diligent about adhering to individual dietary restrictions.

Discharge Planning

Prior to a resident’s discharge from our facility, our rehabilitative professionals make sure they’re ready for the challenges involved with returning home or to an alternate level of care. In addition to outlining personalized exercise plans and guidelines for safely performing daily routines, our therapists will make sure each patient can manage stairs and other obstacles related to returning home. Our social workers may arrange for wheelchair assistance or other assistive devices, and our nutritionists may go over dietary issues that could impact a full recovery. If applicable, our nurses will provide information on wound care and medication management. All this will ensure that once they leave our facility, residents will be adequately prepared to resume optimal functionality and independence.

Social & Recreational Activities

At The Heights we make it a priority to keep residents engaged, interested, and active to the fullest extent possible. Social Services

Upon admission, your social service assistant will begin by completing comprehensive assessments with you. This allows us to best meet the needs particular to you during your stay with us.

Your safe transition home is very important to us. We will begin working with your therapists and nurses as soon as you come to determine what needs you may have when you return home. Establishing a strong support system for your return home is key to a successful discharge. Our social service assistant will provide you with many community resources and arrange services for you prior to your return home. Once you return home, you can expect to receive calls from your social worker to answer any questions or concerns you may have once you have returned home.

During your stay with us we will work very hard to meet the specific needs you have during your stay with us.

What To Bring

To make your stay with us more comfortable and productive, we suggest you bring the following items:

  • Comfortable gym shoes
  • T-shirts or sweatshirts for therapy
  • Loose fitting shorts or pants (for both hip and knee replacements)
  • Comfortable clothes for resting
  • Personal toiletries (we can also provide you with a toothbrush, toothpaste, body wash and razors at no extra charge)
  • Travel case to carry & store items
  • Your driver’s license or picture ID
  • Your insurance and/or Medicare card (or a copy of the front and back of each card)
  • A list of medications you’re taking at home and your pharmacy’s phone number (please leave any medications at home since we’ll provide them for you)
